To name St. Anthony’s Hospital Foundation in your will or trust for a specific amount you may consider the following language:
‘I give and devise to St. Anthony’s Health Care Foundation, Inc. dba St. Anthony’s Hospital Foundation (Tax ID #59-2128991), located in St. Petersburg, FL, all (or state a percentage) of the rest, residue, and remainder of my estate, both real and personal, to be used for its general support (or for the support of a specific fund or program).’
